Hannah’s mantra: “Lead with heart, fight with purpose, and never underestimate the power of small wins.”
Hannah Graham
Senior Support Coordinator | Specialist Navigator
With a lifelong passion for supporting vulnerable communities, Hannah brings over a decade of experience across disability support, child safety, and case management to her role with Incite.
Her journey started when, soon after completing high school, she began working in a specialist school, supporting children with complex needs. It was here she discovered her deep commitment to empowering individuals with disabilities through tailored, person-centred support.
Over the years, Hannah has developed a rich professional background in direct care, community services, and residential child protection before transitioning into NDIS Support Coordination around three years ago. She now specialises in helping participants with systems navigation, with notable strengths in tribunals, complex cases, and systems-based problem solving.
Known for her unique balance of empathy and determination, Hannah is deeply values-driven. She excels in building strong, trusting relationships with participants and their families while fearlessly upholding their rights within often challenging systems.
A natural listener and straight-talker, she’s committed to helping participants not only access the supports they deserve, but also thrive. Whether it’s securing life-changing plan outcomes or celebrating the small wins that build capacity and confidence, Hannah finds purpose in making a difference, one step at a time.
